Kate Moore is the full-time on-site SRS representative at the Wyandotte County Public Health Department. She is an HSS I Bi-lingual in English and Spanish. Kate was hired specifically to support the needs of the Safety Net Clinics for referral and statistical data. 

She provides SRS screening for Medicaid and HealthWave eligibility. Kate also interviews and processes all program applications and provides a myriad of information and referral services regarding all that SRS can provide. Any questions, please contact Kate at 913-573-8815.
