Community Health Improvement Plan
A Community Health Improvement Plan (CHIP) is a long-term, systematic effort to address public health problems based on the results of a Community Health Assessment (CHA). This plan is used by the community, including governmental agencies and community partners, to set priorities, align resources, and coordinate a response.
The CHIP Steering Committee, along with community members and stakeholders, used the CHA findings and further prioritized the community’s health concerns into four strategy areas for the next five years:
- Education and jobs
- Health care access
- Safe and affordable housing
- Violence prevention
Goals, objectives, and strategies have been identified by groups of stakeholders formed to work on each area, and lead agencies have been identified to partner with the Unified Government Health Department in implementing the action plans to ensure this work becomes a reality.
