About the CHIP

Our Collaborative Journey:

Since 2017, the Wyandotte County Public Health Department (WyCo PHD) has provided support for the Community Health Improvement Plan (CHIP), collaborating with community partners and stakeholders to address health concerns identified through the Community Health Assessment (CHA) Process.

Transforming Wyandotte County's Health Together:

Good health goes beyond health care and is impacted by things like the affordability and stability of our housing, the quality of our schools, access to good jobs with fair pay, and the safety of our neighborhoods. Health is defined by the systems and environments we live, work, and age in— environments that can either promote or create barriers to achieving our healthiest lives possible.

A 5-Year Collective Vision:

The Wyandotte County Community Health Improvement Plan (CHIP) focuses on impacting broad social and economic factors that influence the health of our community so that all Wyandotte County residents, neighbors, and families have an equal opportunity to live long and healthy lives. The CHIP is implemented through coordinated efforts between the Wyandotte County Public Health Department, the CHIP Steering Committee and the CHIP Lead Agencies and Action Teams who are dedicated to addressing community-identified priority health areas.

Framed by the CHIP Lenses:

Our plan is guided by an understanding of the challenges we face. Three essential CHIP Lenses – poverty, racism, and ACEs (Adverse Childhood Experiences) – frame our perspective, ensuring our strategies are deeply rooted in the realities of our community.

Collaborative Leadership:

The Wyandotte County Public Health Department (WyCo PHD) is the backbone of the CHIP. With four Lead Agencies specializing in a priority area and the CHIP Steering Committee providing strategic oversight, we coordinate the CHIP Action Teams and their Subcommittees. This collaborative approach ensures that projects are identified, capacity is built, and strategies are implemented effectively.

As we continue to adapt, learn, and grow, one thing remains constant: our shared commitment to a healthier tomorrow for Wyandotte County - and we hope that you will join us on our journey toward the 2024-2028 CHIP!

2018-2023 CHIP

Our Priority Areas

For the 2018-2023 CHIP, The CHIP Steering Committee, community members, and stakeholders used the CHA findings and further prioritized the community’s health concerns into five strategic areas for the next five years. Groups of stakeholders formed to work on each area have identified goals, objectives, and strategies, and lead agencies have been identified to partner with the Unified Government Health Department in implementing the action plans to ensure this work becomes a reality.
