Change your Mailing Address for Property Tax

Take these steps to make changes to your mailing address.

Step 1.Print the Form

Here is the form you will need.

Step 2.Fill out the Form

Please be sure you print all information legibly.  Make sure that you have the notary portion completed if you mail, email or fax the form to us.

Step 3.Attach your Proof of Identification

If you choose to bring the completed form to our office (or another UG office listed at the bottom of the form),  a copy of your picture ID (driver’s license or State ID) is required.  We will photocopy your ID for our records.
