Eligibility Process

Eligibility determination is the first step to accessing services or getting placed on the waiting list for the Home and Community Based Services (HCBS) Intellectual/Developmental Disability (I/DD) Waiver in Wyandotte County.

Eligibility Requirements

Kansas Department of Aging & Disability Services
The Intellectual/Developmental Disability (I/DD) waiver serves individuals aged 5 and older who meet the definition of intellectual disability, having a developmental disability or are eligible for care in an Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF-IID). Those with a developmental disability may be eligible if their disability was present before age 22 and they have a substantial limitation three or more areas of life functioning. 

  • Must be 5 years of age or older.
  • Have Intellectual Disability that began before the age of 18.
  • Have a diagnosis of a Developmental Disability that began before the age of 22.
  • Must be determined program eligible by the Community Disability Determination Organization.
  • Meet the Medicaid long-term care institutional threshold score.
  • Be financially eligible for Medicaid.

Intellectual Disability

Means substantial limitations in present functioning that is manifested during the period from birth to age 18 years and is characterized by significantly sub-average intellectual functioning existing concurrently with deficits in adaptive behavior, including related limitations in two or more of the following applicable adaptive skill areas:

  • Communication
  • Self-care
  • Home living
  • Social skills
  • Community use
  • Self-direction
  • Health & safety
  • Functional academics
  • Leisure
  • Work

Developmental Disability

Means a condition such as autism, cerebral palsy, epilepsy, or other similar physical or mental impairment (or a condition which has received a dual diagnosis of Intellectual Disability and Mental Illness) and is evidenced by a severe, chronic disability which:

  • Is attributable to a mental or physical impairment or a combination of mental and physical impairments, AND
  • Is manifest before the age of 22, AND
  • Is likely to continue indefinitely, AND
  • Results in substantial functional limitations in any three or more of the following areas of life functioning:
    • Self-care
    • Understanding and use of language
    • Learning and adapting
    • Mobility
    • Self-direction in setting goals and undertaking activities to accomplish those goals
    • Living independently
    • economic self-sufficiency
  • It reflects a need for a combination and sequence of special, interdisciplinary or generic care, treatment or other lifelong services, or extended in duration and are individually planned and coordinated, AND

  • Does not include individuals who are solely severely emotionally disturbed or seriously and persistently mentally ill or have disabilities solely due to infirmities of aging.

Children under 6

Developmental disability means a severe, chronic disability that meets all of the following criteria:

  • Is attributable to a mental or physical impairment or a combination of mental and physical impairments, AND
  • Is likely to continue indefinitely, AND
  • Results in at least three developmental delays as measured by qualified professionals using appropriate diagnostic instruments or procedures, AND
  • Reflects a need for a combination and sequence of special, interdisciplinary, or generic care, treatment, or other services which are lifelong or extended in duration and are individually planned and coordinated, AND
  • Does not include individuals who are solely severely emotionally disturbed or seriously and persistently mentally ill.

  • Psychological Evaluation with diagnosis
  • Copy of Medicaid Card (if applicable)
  • Copy of guardianship paperwork (upon request)

  • Copy of Birth Certificate (upon request)

When can I expect information regarding the next steps?

Once we have obtained all the requested information, you will receive a letter within (10) business days. If additional information has been requested or you were referred for an evaluation, additional time may be needed to process your information.

If I am determined eligible for I/DD services, what happens next?

Individuals 5 years and older will need a functional assessment completed within (30) days of eligibility notification. The completed assessment is transmitted to KDADS for State-wide waitlist placement.

Please note: The current waitlist is roughly 5-7 years long.

You may access Targeted Case Manager (TCM) services if you have Title XIX Medicaid or choose to pay for this service privately. If requested, you will receive a choice form with the list of available service providers and be referred within (5-10) days of making that request.

What is your CDDO?

CDDO is the single point of entry for anyone seeking to obtain Intellectual and Developmental Disability services in the State of Kansas. The intake and eligibility process will determine whether a person qualifies for services by collecting necessary documentation such as an application, documentation of the person’s disability meeting the criteria set forth in the DD Reform Act and available school and other records.

Our staff works with you and your family to provide information about available services in your community. Community resources are available for services needed outside of the developmental disability service system.

Kansas currently has 27 CDDO’s throughout the State that serve single and multiple counties performing a gatekeeping role. Currently, we serve only the residents of Wyandotte County.

What is our responsibility as your local CDDO?

We are contracted with the Kansas Department for Aging and Disability Services (KDADS) to perform the following:

  • Work in collaboration and report to KDADS.
  • Develop and implement policies and procedures.
  • Monitor and oversee all affiliated providers.
  • Ensure uniform access to services.
  • Annually provide you with information about all available services and your rights.
  • Establish a Council of Community Members (volunteer committee).
  • Establish a local Quality Assurance Committee (volunteer committee).

Day Services

These licensed services take place during normal working hours. They are regularly occurring activities that provide a sense of participation, accomplishment, personal reward, contribution, or remuneration. They serve as vehicles to maintain or increase adaptive capabilities, independence or integration, and participation in the community. They may be provided in a sheltered workshop, alternative day program, or supported employment.

Residential Services

A licensed service provided to adults ages 18 and over or children ages 5 through 21 who are in voluntary foster care placement.

Residential services are provided outside the family home. Provides assistance, acquisition, and/or improvement of skills related to activities of daily living such as personal grooming and cleanliness, bed making and household chores, eating and food preparation, and the social and adaptive skills necessary to enable the individual to live in a non-institutional setting.

These may be provided in small family-like settings, group homes, and semi-independent or independent living settings.

Case Management Services

Case Management assists the person and the person’s support network to identify, select, obtain, and coordinate services and support. All persons eligible for MR/DD services can request case management services. The person and/or the person’s guardian can choose the provider of case management that they want. They can choose an independent case manager or an agency case manager.

Medical Alert

Council of Community Members (CCM)

K.S.A. 30-64-31 established the Council of Community Members in each CDDO area. The Council of Community Members expresses opinions and makes recommendations to the governing board of the CDDO concerning any service delivery issues.

The CCM consists of volunteer members - a majority of whom are made up of persons with a developmental disability, family members or guardians of a person with a developmental disability receiving services in Wyandotte County and includes affiliates (agencies that provide developmental disability services) of WCDDO and representatives of WCDDO. The CCM meets at least quarterly and at other times as necessary. The meetings are open to the public.

Goal

Provide access to services for persons with developmental disabilities, and coordinate with affiliate agencies and other providers.

Mission

  • Advise the Unified Government on projects, programs, and activities related to people with disabilities.

  • Develop and implement plans and programs to comply with governmental regulations concerning people with disabilities.

  • Provide informational and referral services to people with disabilities and their families.
